Glasgow’s Sandyford Initiative analysed data on 2,500 men who attended a sexual health clinic. One in 10 said they had paid for sex.

Of those around one in four said they repeatedly used prostitutes, 43% had a partner and 20% had a sexually transmitted infection.

Data collected between October 2002 and February 2004 appears in the journal Sexually Transmitted Infections.

The researchers warned that their figures could be an fitness grooming health man sex style of the true numbers who pay for sex.

They found that over half of those men who admitted paying for sex had done so while abroad, while 40% said they had paid for it locally.

Just under 2% said they had paid for sex both in the UK and abroad.

Unprotected vaginal sex was relatively rare, but more common in men who had paid for sex abroad. Unprotected oral sex was more common among the men who paid for sex locally.

And more than half (56%) of men who paid for unprotected sex said that they had a partner.

Similar numbers of men reported paying for sex in saunas and on the streets.

The researchers said this suggests that there needs to be a rethink about health initiatives for sex workers, which have largely been targeted at those involved in street prostitution.

Sexually transmitted infections found among the men who took part in the survey included gonorrhoea, chlamydia, and syphilis. However, none had HIV.

Mental health issues

Peter Baker, chief executive of the Men’s Health Forum, stressed the number of men who paid for sex in the general population - rather than among a group who attended a sexual health clinic - was likely to be much lower.

But he said the figures appeared to confirm a rising trend in the number of men who paid for sex.

“What some do instead is seek respite through alcohol, drugs or sex.”

“There is a need to educate men as well as sex workers about safer sex.

“We have to acknowledge that many men will pay for sex and that they need to know more about condoms, the safety of oral sex, and how to access sexual health services.”

He rarely visited his GP, took little care of his diet and admits his lifestyle was unhealthy and his weight too high.

He says he is delighted with his new image.

“I am one of those men who never went to their doctor unless I could not walk or was seriously ill, and that seemed to be the case of lots of the guys on the course,” he said.

“But now I feel a lot healthier. I am going to the gym four to six times a week.

“The kids are loving it too because they get to go swimming when I go to the gym. The wife has also signed up for the gym and the whole family are now eating much more healthily and taking more exercise.”

Ian, 42, admits that without the programme, which offered a free pass at the local fitness centre as an incentive to shape up, he would still be one of millions of men across the UK a living very unhealthy lifestyle.

But he was far from the only man to get a nasty shock when he finally went along for advice.

“A lot of people who went for the MOT discovered that there was something wrong with them that they did not know about,” he said.

Fish suppers

District charge nurse Jim Leishman and health visitor Alison Dalziel decided to set up the pilot scheme after concerns that men in their area, as in other parts of the country, were not as healthy as they should be.

Ms Dalziel said: “There is nothing specific for men in the area. They were not attending their health services, and in common with most men in Scotland, their health is quite poor.

“We found that a lot of guys who came through the door had not seen their GP for between two to five years.

“I think it is a traditional thing in Scotland to eat unhealthily, with fish suppers and takeaways readily available. And when it is pouring down who wants to go out for a walk to get some exercise?

“Mental health problems also get ignored because you are seen as a wimp if you cry.”

She said that the weight management courses had proved particularly popular, and that all the men had been very enthusiastic.

“The 10 men on the first course lost 13 stone between them and 44 inches from their waists.

“They all did well and were all very enthusiastic.”

Proactive approach

Mr Leishman, who ran the initial course, said almost 650 men had now attended the clinic.

It was quickly apparent that many of the men had problems with stress, weight problems or blood pressure.

Just under three quarters of the men who attended were overweight or obese, 57% had high blood pressure, 38% had mental health problems, 23% presented with urinary problems and 11% had some form of erectile man s health magazine

Not only did the clinic deal with health problems which were already apparent, they also took a proactive approach, discussing with the men how best they could prevent further trouble from occurring in the future.

The team has also developed a course to help practitioners - mainly nurses so far - to set up services aimed at improving men’s health.

Some said they were embarrassed, while others said they were afraid of losing their masculinity.

The research in the British Journal of General Practice followed the experiences of 45 men from suspecting something was wrong to diagnosis.

The authors said men should be aware the cancer can be successfully treated.

Doctors based at the University of Oxford interviewed men diagnosed with testicular cancer between August 2001 and February 2002.

They were asked to tell their stories from when they first suspected they had a problem, and to explain in detail why any delays had occurred.

In the majority of cases, the men had either found a lump on their testicle, or the testicle had become swollen or painful.

But some men put off going to see their GP for weeks or even months.

Some were not in any pain and a few had no obvious lump, but others 39 health magazine man s

Others said they were worried about appearing weak, being seen as a hypochondriac, and losing their masculinity.

Some men also said they were embarrassed about going to see a doctor.

Many were finally prompted to go to their GP after seeing articles about testicular cancer in the media.

The researchers say their findings also showed a diagnosis of testicular cancer could at times difficult to make.

They are calling on GPs to be extra cautious with men who have testicular complaints, and to have a low threshold for sending them for ultrasound investigations of symptoms.

Targeting advice

Dr Ann McPherson, a GP who co-wrote the report, said: “It is important that men know the signs and symptoms of testicular cancer, but equally important that they understand that a diagnosis of testicular cancer is not a death sentence and that the cure rate is excellent.

“Men also need easy access to doctors, at surgeries where they feel welcome, and to clinics where they can seek advice without embarrassment.”

Dr Ian Banks, chair of the Men’s Health Forum, told BBC News Online: “Some young men wait up to 14 weeks after finding a lump on their testicle before going to the GP.

“I’d like to think things are getting better in terms of men looking after their health. But there seem to be areas where we’re no better than we were 20 years ago.”

The ‘Time to Go’ campaign has set up a telephone helpline for men who think they may have prostate problems.
