Newswise — The decreased use of cocaine in the United States over the last 20 years mostly occurred among the highly educated, while cocaine use among non-high school graduates remained constant, according to a study by researchers at the Johns Hopkins Bloomberg School of Public Health. The study authors suspect that the inverse relationship between cocaine use and education is related to access to health warnings and resources. They also concluded that the emerging disparity highlights the need for improved interventions that target persistent cocaine users who are lower educated. The study is published in the October 2007 issue of the American Journal of Public Health.

Much like smoking, people with a better understanding of the impact cocaine has on health are more likely to modify their behavior, said Valerie S. Harder, MHS, lead author of the study and a PhD-candidate in the Bloomberg School of Public Healths Department of Mental Health. Better educated individuals also may have more resources and access to health care services, such as drug treatment programs.

The researchers used data from the 1979-2002 National Survey on Drug Use and Health to compare cocaine use and educational achievement for adults aged 19-50 years. Adults were categorized as non-high school graduate, high school graduate or college graduate and as either recent-onset or persistent cocaine users.

The proportion of non-high school graduates using cocaine has remained consistent from 1979-2002. Early data suggests that high school and college graduates were more likely to start and persist in their use of cocaine, as compared to non-high school graduates. During the late 1980s, the proportion of high school and college graduates classified as persistent users dropped dramatically and fell below that of non-high school graduates. During the same time period, first time cocaine use steadily decreased among all adults regardless of their level of educational achievement and remained low.

It isnt enough to simply try to stop individuals from using cocaine the first time, said Harder. More drug intervention programs that target non-high school graduates are necessary to reduce persistent cocaine use in that population.

A new effort is being made to get teenagers learning “1066 and all that”.

England’s Qualifications and Curriculum Authority (QCA) has produced guidance stressing the use of chronology in history to build up “the big picture”.

It also aims to encourage learning about change and continuity - identifying themes such as how events affected people’s working lives.

QCA chief executive, Ken Boston, said: “The dates of key events in history are an important part of learning.”

Timelines

He added: “Children need to know when things happened but also why they matter to the lives they lead today and the events that helped to shape our world.”

Last year the then Education Secretary, Charles Clarke, asked the QCA to review the history curriculum - which is compulsory in England only up to the age of 14.

In an interview, he said: “A lot of people - people I respect - say that there is not enough of a sense of a timeline in history, so that pupils have too much detailed study of particular eras and not enough of a sense of context for what happened.

“That’s not simply an appeal to go back to dates and kings and queens and that sort of thing but it is saying that by the time they are 14, children should have a pretty good appreciation of the history of this country, of Europe, and the context within which things happened.”

Prince Charles had also complained about people leaving school without “valuable and essential knowledge and understanding about their national history and heritage”.

Ecstatic peasants

Among the classroom activities suggested in the QCA’s new guidance are “timelines with attitude”, requiring pupils to place events on a timeline which acts as the horizontal axis of a graph while the vertical axis is used to describe feelings.

So they might imagine being peasants between 1348 and 1381, veering in mood from ecstatic to despairing, it says.

One of the suggested ideas that pupils in their first year of secondary school might explore is: “Did the Romans do more harm than good?” - which inevitably conjures up images of the “What did the Romans ever do for us?” scene in Monty Python’s Life of Brian.

A spokesman for the QCA said: “To learn history well you need some basic knowledge, including dates, but what brings history to life is the impact that those dates still have on the world that we live in.”

On the pivotal year 1066, the guidance says: “Pupils cannot be expected to understand the concepts of causation, or change and continuity, unless they can put events in the correct chronological order.”

An exercise might be to draw on the following sentences to write a paragraph explaining what happened:

• A great battle was fought.
• Harold was crowned King of England.
• King Edward died.
• The King of Norway invaded England.
• Another great battle was fought.
• The Duke of Normandy invaded England.

A question might be: “Why would this task be easier if you had been given the date of each event?”

And: “If the six sentences are in the wrong order, why does this make it difficult to explain why the Duke became King?”
The book 1066 And All That, by WC Sellar and RJ Yeatman, published in 1930, was a joke at the expense of history teaching in England at the time.

Famous for its catchphrases “A Good Thing” and “A Bad Thing”, fun test papers in the book included the “memorable” instruction: “On no account attempt to write on both sides of the paper at once.”

U.S. politicians on Thursday treated
the collapse of a highway bridge that killed or injured dozens
of people as a jarring wake-up call to fix the nation&39;t fall down,” said Sen.
Amy Klobuchar, a Minnesota Democrat at a news conference in
Minneapolis. “We have to get to the bottom of this.”

“We should look at this tragedy that occurred as a wake-up
call for us,” said Senate Majority Leader Harry Reid, a Nevada
Democrat. “We have all over the country a crumbling
infrastructure; highways, bridges and dams. We really need to
take a hard look at this.”

Rep. James Oberstar, the Minnesota Democrat who chairs the
House Transportation and Infrastructure Committee, blamed
President George W. Bush&36;2 billion a year for bridge reconstruction when lawmakers were
pushing for &39;re not going to settle for a bargain-basement
transportation” policy, Oberstar said.

The problem of aging infrastructure is not new. A 2002
report by the Department of Transportation said about 30
percent of the nation&39;s bridges were deteriorating with
age and growing traffic volumes were increasing the strain on
them.

ALMOST FAILING &39; GRADE

A 2005 report by the American Society of Civil Engineers
gave the country&36;1.6 trillion over five years to put its infrastructure
into good shape.

“This has been out there for quite some time,” said Kent
Harries, an engineering professor at the University of
Pittsburgh. “It&39;s aviation system, dams, drinking
water, electric power grid and hazardous waste system.

Robert Dodds, head of the engineering department at the
University of Illinois at Urbana-Champaign, said what made the
bridge collapse so shocking was the general reliability of
bridges nationwide.

“We take their safety for granted every day,” he said.

But Harries said infrastructure failures happen more
frequently than most people notice, pointing to the collapse of
a concrete bridge box girder near Pittsburgh in 2005 and the
recent explosion of a steam pipe in Manhattan. Part of the
problem is finding maintenance funds.

“We recognize that there is a problem but there just seems
to be this inability to move on it, partially I suspect because
the problem is so amazingly large. The dollar values that we’re
talking about, they defy understand,” Harries said.

Funding it all would require trillions of dollars. The only
way to address the issue is to prioritize, he said, but then
politics comes into play.

“The fact of the matter is nobody gets their name on a
bridge repair,” Harries said. “You build a bridge, you get your
name on it.”

Newswise — Neurons, or nerve cells, communicate with each other through contact points called synapses. When these connections are damaged, communication breaks down, causing the messages that would normally help our feet push our bike pedals or our mind locate our car keys to fall short.

Now scientists at the University of North Carolina at Chapel Hill School of Medicine have shown that a protein called neurexin is required for these nerve cell connections to form and function correctly.

The discovery, made in Drosophila fruit flies may lead to advances in understanding autism spectrum disorders, as recently, human neurexins have been identified as a genetic risk factor for autism.

“This finding now gives us the opportunity to see what job neurexin performs within the cell, so that we can gain a better insight into what can go wrong in the nervous system when neurexin function is lost said Dr. Manzoor Bhat, associate professor of cell and molecular physiology in the UNC School of Medicine and senior author of the study.

The study, published online September 6, 2007, in the journal Neuron, is the first to successfully demonstrate in a Drosophila model the consequences that mutating this important protein may have on synapses.

The research was supported in part by grants from the National Institute of General Medical Sciences, National Institute of Neurological Disorders and Stroke and the National Institute of Mental Health and funds from the state of North Carolina.

During the last decade, scientists have learned that neurexins are integral to the transmission of chemical signals within the nervous system. Neurexins interact with binding partners called neuroligins to link neighboring nerve cells together so that signals can be sent and received correctly.

Previous attempts to study these proteins in animal models have been challenging. In vertebrates such as mice, three different genes code for the production of certain neurexin proteins. Deleting just one of these genes causes no adverse effects in mouse models, while removing all three is fatal. But fruit flies have only one gene for neurexin, and when Bhat and colleagues deleted the gene, the flies survived barely.

“Knocking out neurexin basically resulted in a fly with defective nervous system said Bhat, also a member of the UNC Neuroscience Center and the UNC Neurodevelopmental Disorders Research Center.

First of all, the mutated fruit flies had trouble moving around. When the researchers examined the synapses in these flies, they found that half of them were gone. The synapses that remained were deformed, causing them to send out less chemical signals. The researchers, led by Jingjun Li, a graduate student in neurobiology in the UNC School of Medicine, concluded that neurexin is required for the growth of synapses, for the maintenance of their structure and for their function.

Currently, Bhat and other scientists are working to identify the proteins that neurexin binds to, how they interact, and what sequence of events ultimately results in the organization of synapses within nerve cells. The hope is that such studies in Drosophila will one day clarify the role neurexin plays in learning and memory, ultimately leading to a better understanding of how defects in this protein can lead to human disorders such as autism, Bhat said.

Study co-authors include James Ashley and Vivian Budnik from the University of Massachusetts Medical School.

Young adults may be being prescribed unnecessary medication for high blood pressure, researchers have warned.

A study found doctors were more likely to misdiagnose hypertension - high blood pressure - in under 35s than to diagnose it correctly.

Writing in the British Medical Journal, researchers from Birmingham University said the mistake could mean patients were put on lifelong medication.

They urged doctors to diagnose the condition with caution in young adults.

Guidelines recommend that doctors routinely check all their patients’ blood pressures.

But Dr Tom Marshall, a public health expert, said natural variations in readings meant high blood pressure could be over-diagnosed.

Dr Marshall studied the blood pressure findings of more than 13,000 adults whose blood pressure had been taken each year over three years as part of a larger study into cardiac disease risk factors.

He then analysed the data to calculate how many were truly hypertensive.

He found that, of 36 men aged 16 to 34 who had been classified as needing treatment, only 11 really needed treatment.

Of 19 women, only five actually needed to be on medication to lower their blood pressure.

‘No symptoms’

Dr Marshall said young adults were at low risk of heart disease and there was little benefit from treatment.

He said hypertension in young adults should be diagnosed with caution by taking the
average of many blood pressure measurements or even using a higher threshold than is used now.

He added: “In adults under 35 clinicians should diagnose hypertension with caution.

“Blood pressure measurement is useful mainly in people with specific indications or coronary risk factors.”

Belinda Linden, of the British Heart Foundation, said: “Treatment for high blood pressure is generally not considered until someone has been found to have a consistently raised blood pressure on three consecutive occasions over a number of months. This should ensure that treatment is not started unnecessarily.

“High blood pressure often has no obvious symptoms. Measurement is the only way to ensure appropriate treatment is given and the risk of coronary heart disease in later life is avoided.

“Hypertension is more prevalent in older people, but with the growing problem of obesity and inactivity, it is possible that high blood pressure will increasingly affect younger people.

“For that reason, it is important to remain vigilant in checking blood pressure levels.”

Science Daily — A University of Alberta researcher has discovered a potential breakthrough for premature ejaculation–the most common sexual dysfunction in men–with a drug usually used to treat bi-polar or anxiety disorder.

Dr. Pierre Chue, a psychiatry professor at the U of A, has found success in treating premature ejaculation (PE) with the use of gabapentin, better known by the brand name Neurontin. Chue writes about his findings in the September issue of the “Canadian Journal of Psychiatry.”

“This disorder affects almost 40 per cent of males–it is even more common than erectile dysfunction–yet it is not talked about much and there has been very little research on it,” said Chue.

The essential feature of the disorder is persistent ejaculation with minimal sexual stimulation before or shortly after penetration and before the person wishes it. PE is believed to be a neurobiological phenomenon involving primarily a disturbance of serotonin receptor function. Currently, physicians prescribe medications that are known to influence these receptors–selective serotonin reuptake inhibitors or SSRIs–that delay ejaculation but these antidepressants also come with negative side-effects..

In his report, Chue cites a case study in which a 40-year-old man diagnosed with PE received minimal effectiveness from different techniques–the use of a condom with topical anesthetic and different antidepressant drugs–aimed to improve the disorder. The drugs resulted in such side effects as restless legs, headaches, decreased libido or accelerated ejaculation. The man “had previously found that alcohol produced satisfactory ejaculatory delay with no loss of erectile capacity, but clearly this was not a feasible regular option,” says Chue. A trial of gabapentin taken one to two hours before intercourse proved effective. Higher doses prolonged ejaculation even further but also caused drowsiness.

Dr. Chue is not certain how gabapentin works to improve PE but believes it has to do with the drug’s ability to increase aminobutyric acid (GABA), the most important inhibitory neurotransmitter in the brain. Since there are currently no specific treatments for PE, the use of gabepentin to prolong ejaculation warrants further study, says Chue, particularly for those men where other therapies are ineffective or poorly tolerated.

Meanwhile, Chue is looking for people to participate in a clinical trial he is running that will use an SSRI-type drug called dapoxetine, to learn its effects on men with PE. This is an SSRI with a very short half-life that has been shown in clinical trials to delay ejaculation without the usual SSRI side effects.

MARKHAM, Ont. (CP) - The strong Canadian economy is helping to offset the financial storm that has swept through global credit markets, Finance Minister Jim Flaherty said Sunday.

Flaherty told the Canadian Chamber of Commerce that solid jobs growth in the economy, government budget surpluses and falling debt burden as well as the country’s role as an emerging energy superpower have helped ease the economic stresses caused by the credit crunch.

“”The Canadian economy is the strongest it has been in a generation,”" Flaherty told the annual meeting of the Chamber, the country’s biggest business lobby group with 170,000 members. “”That fact, along with important initiatives taken by the Bank of Canada and the private sector have supported the functioning of the markets.”

While part of Canada’s asset-backed commercial paper market - a system of short-term corporate lending - has been frozen, the Canadian financial system has been generally functioning normally. A plan has been proposed to freeze and eventually restructure the $35 billion market for asset-backed commercial paper that is not sponsored by the big banks.

The Bank of Canada and other central banks around the world have pumped billions of dollars into the financial system in recent weeks to help ease the credit turmoil. But troubles persist. In recent days, hundreds of customers lined up outside branches of a British mortgage bank, Northern Rock, to withdraw their savings after the bank asked for an emergency Bank of England loan.

The British bank made the request because it relies heavily on wholesale money markets for cash, and had been unable to borrow the amounts it needed from other banks since money markets choked up last month. That was caused in part by U.S. banks making mortgage loans to Americans with poor credit histories. The fallout from that lending has led to rising foreclosures in the U.S. housing market, the near collapse of some U.S. mortgage lenders and the seizing up of financial markets linked to risky mortgage lending.

The main problem appears to be that hedge funds, investors and other financial players can’t properly value the complex, poor-quality financial products they hold that are linked to the U.S. sub-prime mortgage market.

Earlier this week, some of the chief executives of Canada’s big banks said they expect the global credit turmoil to last until next spring at least. Flaherty agreed.

“”Clearly, the storm has not yet subsided, but I can assure you our government and the Bank of Canada will continue to monitor the situation closely,”" he told the Chamber, which is meeting in this community just north of Toronto.

Flaherty met with the CEOs of the big Canadian banks a few days ago and later praised the plan proposed by the Caisse de depot for a 60-day freeze that will help financial companies properly value the asset-backed securities they hold. As well, Bank of Canada Governor David Dodge said the private sector needs to take the initiative to work out the problems in the credit squeeze and not expect central banks to bail them out.

“”The guiding principle must be caveat emptor (buyer beware),”" Dodge told a business group in London England.

In his speech Sunday, Flaherty also told the Chamber that Canada faces some economic challenges, which the Conservative government is trying to address with its tax and spending policies:

-The rising Canadian dollar and its impact on the manufacturing sector, especially in Ontario and Quebec.

-Increased competition from emerging economic giants like China, India and Brazil. China, the world’s fastest growing economy, has just surpassed Canada as the leading exporter of goods to the United States.

-A shortage of skilled workers and an aging population as well as outdated and inadequate infrastructure.

“”The world economy is changing and Canada needs to adapt to it,”" Flaherty said. “”We need to look further ahead to sharpen our competitive edge. Our objectives are clear: more and better jobs, a higher standard of living, and greater opportunity for Canadians to learn, earn, and invest to create the future they want for themselves and their families.”"

SATURDAY, Sept. 29 (HealthDay News) — People with heart disease
should not stop taking cholesterol-lowering drugs before any kind of
noncardiac surgery, say new joint guidelines released this week by the
American Heart Association and the American College of Cardiology.

The new guidelines, which update those published in 2002, provide a
framework for considering and reducing a heart disease patient’s risk of a
cardiac event during or immediately after non-heart-related surgery.

Many heart disease patients can safely undergo noncardiac surgery
without first “fixing” their heart disease with coronary bypass grafting
or an artery-opening procedure, state the guidelines. They also outline
how to treat patients who do need a heart procedure before noncardiac
surgery, have coronary stents, or require anti-clotting medication. More…

Almost 500 workers have been offered anti-viral drugs following the outbreak of avian flu on the Bernard Matthews poultry farm in Holton, Suffolk.

The Health Protection Agency (HPA) said the 480 people included workers and those involved in the clean-up.

Six people who developed flu-like symptoms underwent tests, but tested negative for avian flu, the HPA said.

It added it was continuing to monitor the situation locally, working with Defra and the Bernard Matthews company.

Nearly 160,000 turkeys were culled at the firm’s Holton plant after the potentially fatal H5N1 strain of bird flu was discovered on 3 February

The HPA said that as a precautionary measure, a small number of workers involved in the cleaning and decontamination process were still taking anti-viral medications.

Dr Jonathan Van Tam, a flu expert at the Agency, said: “The risk of any workers testing positive for avian flu has been, and remains, very low as they have followed all the necessary precautions in terms of protective clothing and hygiene measures, and have been offered antiviral drugs.”
